Cannabidiol, veiligheid, effect, bijwerkingen en interacties

Wat is het?
Cannabidiol is een chemische stof in de Cannabis sativa plant, ook wel bekend als marihuana. Meer dan 80 chemische stoffen, bekend als cannabinoïden, worden gevonden in de Cannabis sativa plant. De belangrijkste is Delta-9-tetrahydrocannabinol (THC). Cannabidiol is ongeveer 40% van de cannabisextracten en is onderzocht voor vele verschillende toepassingen.

Waarom gebruikt men het?
Mensen gebruiken cannabidiol oraal voor angst, bipolaire stoornis, een spierziekte genaamd dystonie, epilepsie, multiple sclerose, ziekte van Parkinson en schizofrenie.

Mensen inhaleren cannabidiol om te helpen stoppen met roken.

Een receptplichtige neusspray (Sativex, GW Pharmaceuticals) die zowel THC en cannabidiol bevat, wordt gebruikt voor de pijn en spier-dichtheid bij mensen met multiple sclerose in meer dan 25 landen.

Effectiviteit

Multiple sclerose (MS). Deze is effectief gebleken voor het verbeteren van de pijn, spier-dichtheid en plas frequentie bij mensen met MS. Maar er inconsistente gegevens over de effectiviteit van cannabidiol op symptomen van multiple sclerose wanneer het alleen wordt gebruikt. Sommig vroeg onderzoek suggereert dat het gebruik van cannabidiol onder de tong pijn en spierstijfheid kan verbeteren, maar niet spierkrampen, vermoeidheid, controle over de blaas, mobiliteit, of het welzijn en de kwaliteit van leven bij patiënten met MS.

Het effect van Cannabidiol bij de volgende toepassingen is onduidelijk, negatief of tegenstrijdig. Dit is wat er tot nu toe bekend is:
Bipolaire stoornis. Onderzoek lijkt aan te geven dat het dagelijks nemen van cannabidiol de manische episodes bij mensen met een bipolaire stoornissen niet verbeterd.

Een spierziekte genaamd dystonie. Vroeg onderzoek toont aan dat het nemen cannabidiol iedere dag gedurende 6 weken dystonie kan verbeteren met 20% tot 50% bij sommige mensen. Maar het onderzoek was niet van goede kwaliteit en voordat er conclusies getrokken kunnen worden moet kwalitatief beter onderzoek gedaan worden.

Epilepsie. Het nemen van cannabidiol daags gedurende maximaal 18 weken zou aanvallen bij sommige mensen verminderen. Maar ander onderzoek toont aan dat het nemen van cannabidiol iedere dag gedurende 6 maanden niet helpt. Redenen voor de tegenstrijdige gegevens zijn onduidelijk. Misschien waren de studies te klein.

De ziekte van Huntington. Cannabidiol verbetert de symptomen niet

Slapeloosheid. Onderzoek suggereert dat het nemen van cannabidiol 160 mg voor het slapen helpt om langer te slapen bij mensen met slapeloosheid. Maar lagere doseringen hebben dit effect niet. Cannabidiol Het lijkt niet te helpen om in slaap te vallen en vermindert het vermogen om dromen te herinneren.

Ziekte van Parkinson. Onderzoek toont aan dat het nemen van cannabidiol daags gedurende 4 weken de psychotische symptomen bij mensen met de ziekte van Parkinson en psychose verbetert. Maar dit specifieke cannabisextract (Cannador) bevat THC en cannabidiol en lijkt geen effet te hebben op de onwillekeurige spierbewegingen veroorzaakt door anti-Parkinson geneesmiddel levodopa.

Schizofrenie. De resultaten bij onderzoek naar het gebruik van cannabidiol en psychotische symptomen bij mensen met schizofrenie gemengd. De resultaten waren in sommige gevallen positief en in sommige gevallen negatief.

Stoppen met roken. Het inademen van cannabidiol met inhaler gedurende een week verminderde het aantal gerookte sigaretten met ongeveer 40%.

Sociale angststoornis. Het nemen van cannabidiol 300 mg per dag heeft geen effect op angst bij mensen met een sociale angststoornis. Ander onderzoek zegt dat het nemen van een hogere dosis (400-600 mg) de angst geassocieerd met het spreken in het openbaar vermindert.

Hoe werkt Cannabidiol?


Cannabidiol heeft antipsychotische effecten. De precieze oorzaak van deze effecten is niet duidelijk. Maar cannabidiol lijkt de afbraak te voorkomen van een chemische stof in de hersenen die pijn, stemming en mentale functie beïnvloedt . Het voorkomen van de afbraak van deze stof en verhoging van de gehaltes in het bloed lijkt psychotische symptomen geassocieerd met aandoeningen zoals schizofrenie te verminderen. Cannabidiol blokkeert ook sommige van de psychoactieve effecten van delta-9-tetrahydrocannabinol (THC). Cannabidiol lijkt pijn en angst verminderen.

Bijwerkingen en veiligheid van Cannabidiol


Cannabidiol is mogelijk veilig bij inname via de mond en op de juiste wijze bij volwassenen. Cannabidiol doseringen tot 300 mg daags zijn veilig voor maximaal 6 maanden. Hogere doses van 1200-1500 mg daags zijn veilig voor maximaal 4 weken. Cannabidiol sprays onder de tong zijn veilig in doses van 2,5 mg tot 2 weken.

Sommige gemelde bijwerkingen van cannabidiol zijn een droge mond, lage bloeddruk, licht gevoel in het hoofd, en slaperigheid.

Bijzondere voorzorgen en waarschuwingen:
Zwangerschap en borstvoeding: Er is onvoldoende betrouwbare informatie over de veiligheid van het nemen van cannabidiol als u zwanger bent of borstvoeding geeft. Gebruik het dus niet.

De ziekte van Parkinson: Sommig onderzoek suggereert dat het nemen van hoge doses cannabidiol spierbewegingen en tremoren erger kunnen maken bij mensen met de ziekte van Parkinson.

Zijn er interacties met medicijnen?


Wees voorzichtig met deze combinaties:
Medicijnen die afgebroken worden door de lever of de lever beinvloeden:
Cannabidiol zou de snelheid waarmee de lever sommige medicijnen afbreekt kunnen verminderen en daarmee de bijwerkingen van deze medicijnen kunnen verhogen. Wanneer u medicijnen gebruikt kunt u beter eerst overleggen met de arts voor u cannabidiol neemt.

amitriptyline (Elavil), haloperidol (Haldol), ondansetron (Zofran), propranolol (Inderal), theofylline (Theo-Dur, anderen), verapamil (Calan, Isoptin, anderen), en anderen.
(cytochroom P450 1B1 (CYP1B1) substraten)
theofylline (Theo-Dur, anderen), omeprazol (Prilosec, Omesec), clozapine (clozapine, FazaClo), progesteron (Prometrium, anderen), lansoprazol (Prevacid), flutamide (Eulexin), oxaliplatin (Eloxatin ), erlotinib (Tarceva), en cafeïne.

(cytochroom P450 2A6 (CYP2A6) substraten)
nicotine, chlormethiazole (Heminevrin), coumarine, methoxyfluraan (Penthrox), halothaan (Fluothane), valproïnezuur (Depacon), disulfiram (Antabuse), en anderen.
(cytochroom P450 2B6 (CYP2B6) substraten)
ketamine (Ketalar), fenobarbital, orfenadrine (Norflex), secobarbital (Seconal), en dexamethason (Decadron).
Medicijnen veranderd door de lever (cytochroom P450 2C19 (CYP2C19) substraten)
protonpompremmers, waaronder omeprazol (Prilosec), lansoprazol (Prevacid) en pantoprazol (Protonix); diazepam (valium); Carisoprodol (Soma); nelfinavir (Viracept); en anderen.
(cytochroom P450 2C9 (CYP2C9) substraten
niet-steroïdale anti-inflammatoire geneesmiddelen (NSAIDs), zoals diclofenac (Cataflam, Voltaren), ibuprofen (Motrin), meloxicam (Mobic) en piroxicam (Feldene); celecoxib (Celebrex); amitriptyline (Elavil); warfarine (Coumadin); glipizide (Glucotrol); losartan (Cozaar); en anderen.
 (cytochroom P450 2D6 (CYP2D6) substraten
amitriptyline (Elavil), codeïne, desipramine (Norpramin), flecaïnide (Tambocor), haloperidol (Haldol), imipramine (Tofranil), metoprolol (Lopressor, Toprol XL), ondansetron (Zofran), paroxetine (Seroxat ), risperidon (Risperdal), tramadol (Ultram), venlafaxine (Efexor), en anderen.
(cytochroom P450 3A4 (CYP3A4) substraten)

Sedatieve medicijnen (CNS depressiva)
Cannabidiol kan slaperigheid veroorzaken. Medicijnen die slaperigheid veroorzaken, worden kalmerende middelen genoemd. Inname van cannabidiol samen met kalmerende medicijnen kan teveel slaperigheid veroorzaken.

Sommige kalmerende medicijnen zijn onder andere benzodiazepinen, pentobarbital (Nembutal), fenobarbital (Luminal), secobarbital (Seconal), thiopental (Pentothal), fentanyl (Duragesic, Sublimaze), morfine, propofol (Diprivan), en anderen.
Kruiden en supplementen met kalmerende eigenschappen
Cannabidiol kan slaperigheid of sufheid veroorzaken. Er zijn behalve medicijnen ook kruiden en supplementen die dit doel kunnen bereiken. Deze 2 samen nemen kan teveel slaperigheid veroorzaken, bijvoorbeeld kattenkruid, hop, Jamaicaanse kornoelje, kava, L-tryptofaan, melatonine, salie, zelfde, sint-janskruid, sassafras, en anderen.

Voor multiple sclerose: Een spray levert 2,5 mg cannabidiol onder de tong per dosis is gebruikt.
